I confirm that I am requesting McKeevers Chemists to return my original prescription to the return address provided using DPD. I confirm that the name, return address and prescription address details I have provided are accurate and relate to the prescription I am asking to be returned. I understand that once the prescription has been handed over to DPD, responsibility for the safe delivery of the prescription passes to DPD and/or to me as the customer. I understand that McKeevers Chemists cannot be held responsible if the prescription is lost, delayed or damaged in transit after it has been handed over to DPD. I understand that the pharmacy will retrieve and return the relevant prescription or prescriptions as soon as reasonably possible after my request and payment have been received.
